Certificate of the operational registration

We issue an extract of the valid operational registration at the request of clients, in Hungarian and/or English.

To obtain an extract, you need to submit the following documents:

• filled out and signed application form
  • application form for doctors, dentists and pharmacists
  • application form for healthcare workers (physiotherapists, nurses, midwives, etc.)
• copy of the valid passport/ID card
• valid membership of the chamber (if it is compulsory)

The application can be sent:

• by post to P.O. Box 270, H-1444 Budapest, or
• by e-mail to omn@okfo.gov.hu, or
• in person at our customer service during office hours.

The procedure is free of charge.
