The request can be submitted only on this application form.

The application form and the supporting documents can be sent:

  • by post to P.O. Box 270, H-1444 Budapest, or
  • in person at our customer service during office hours.

Please note that in accordance with the recognition procedure only the following documents can be considered as a certified translation and a certified copy:

Certified translation

  • a certified translation made by the Hungarian Office for Translation and Attestation
  • a translation bearing the verification clause of a Hungarian consulate
  • a translation bearing the verification clause of a Hungarian public notary
  • a translation considered to be certified by the legislation of a Member State

Certified copy

  • a copy considered to be certified by the national law (e.g a certified copy made by a public notary)
  • a copy made by a consultant of the National Directorate General For Hospitals (NDGFH) - in case of personal submittance of the application, during the opening hours of the NDGFH (used in the procedure of the NDGFH only)
  • a certified copy of a foreign language document made by the Hungarian Office for Translation and Attestation
  • a copy considered to be certified according to the legislation of a Member State
